I'am Updating my U4 Firmware from 17.5 - V-20180417 can I go direct to Version 19.3 - 20191022 ?

Neither 20190207 or 19.3 -20190630 OR 20190818.were ever installed.

Does this also include unplugging and removing the USB in the USB-Tuner Loop

You should remove the USB tuner's USB connection. It doesn't matter whether the tuner's antenna connection stays connected or not.

If you're flashing firmware via USB, then it's a complete replacement so in that regard it doesn't matter what was there previously.
The settings backups aren't always compatible between different firmware series (sometimes versions too). In your case, the settings backup taken with 17.5-series can be used to restore the settings in the 19.3-series firmware, so make sure you take a backup just before you flash with the new firmware.

I've never needed to remove the USB tuner connection from the U4 when updating the firmware via USB.
I flashed the U4 twice yesterday with the USB tuner still connected.
I also had a USB external drive attached too

Pay attention to the front panel of the U4 when you reboot with the USB firmware thumb drive attached, as the TV screen won't tell you what to do, only the front panel does and you've only got about 8 seconds to press the front panel power button.
